No. Type a couple of lines if you prefer. Either way the report is written from what you said, not from a template.
Can the customer save it as a PDF?
Yes. They open the link and can save or print it from their phone or computer. No account and no app needed.
What if I forget to mention the cause?
Then the report won't state one — it won't guess. Add a note with the detail and regenerate, or type it into the report yourself.
